| All visits with Dr. Moore are arranged by appointment to allow
adequate time for each patient.
Please bring with you the names, addresses, phone numbers of other
physicians involved in your care with whom we may need to correspond. In
addition, please bring copies of hearing tests, ENGs and CT/MRI scans
which may have previously been done to evaluate your ear problem.
In order for Dr. Moore to complete your exam, you may need a hearing
test before seeing the doctor. This test should be scheduled with your
initial appointment.
