Payment for office services is expected at the time of the visit. For your convenience, we accept VISA and MasterCard.Hospital related services and operation fees will generally be submitted to your insurance carrier, although you are ultimately responsible for payment. The itemized receipt you receive at each office visit will satisfy the needs of most insurance companies. If you have an HMO insurance plan, it is imperative that you bring your referral form with you as well as money for your copay as applicable. If you do not bring a current referral form, you will be responsible for payment of your bill in full.
